I noticed it on a few engines and it drives me mad. It's pulling away in first and the revs drop a bit. I once tried adjusting a slide but that didn't work. Once into 2nd etc it's fine. Set up at the minute is a GT186 (best smallframe Kit, IMO) 25 Dellorto and Clubman. Gearbox li125 16x46.

Carburation is probably too rich at full revs. Not noticeable in higher gears because engine load doesn't allow it to rev so high.

Is it just as you twist the throttle , like its gonna stall or run out of petrol ?

Yes it's worse when cold and it feels carb related. As I say, I noticed on a few scooters.

^ yes  but it seems a carb adjustment would be possible but where and how? I did reduce the angle of a slide once but it didn't make any difference.

Those symptoms can be the airscrew causing it be lean or the pilot jet is to small

I agree with shocky on this one. To test it try pulling away with choke on.
