Cosa clutch wear

Just after opinions on this wear:

Shite picture, but it is angled/worn in towards the centre.

Pictures hard to see but at a guess the nut was probably not done up tight enough and or fat thrust washer missing :roll:

Chers guys,
Not sure on the cause, when I stripped it, the clutch washer/oil drive was in 6 pieces, and the clutch was worn like this.
I didn't know which would have caused what first, and if it was something people had seen before.
Clutch was tight and oil was ok.

Hopefully was just a faulty washer, or some animal well overtightened the clutch in the past :?
