Jumping out of 2nd & can't select 4th gear

After finally getting it roadworthied & rego, now starting to run it in, tends to jump out of gear in second with a big distance on the shift between 1st & second, can't select 4th. initially found that the arm was fowling on the spiggot bolt, sorted that, got it into fourth on the stand, adjusted it from there back. Thinking maybe got 4th gear in wrong way (would that cause it) cruciform maybe worn as I used the original or possibly shimming not quite right...anyone else had a similar prob after total rebuild?
cable adjustment? cables in the right way round? I'd try the easy stuff first before you do a full autopsy
I am playing all the right notes, but not necessarily in the right order.

Think the shimming would have to be way off for the selection of 2nd to become impossible.
Is the gear arm that fits to the tie bar in the correct position? It can be incorrectly fitted so it fouls on the damper lug when you try to get it into certain gears.
After one rebuild I could not select gears......turned out I had trapped a gear cable inner behind the damper nut. Daft but I managed it.
I'd look at all this and more before pulling it apart again.

Had it apart today, gears were assembled correct but cruciform is slightly worn...plus mainly not shimmed out enough, so thinking that the combination of the two is more likely the problem, although I have some shims, will order intermediate sizes just in case & a new cruciform.
