jumping out of gear

Anyone have any suggestions on why this might happen. it knocks out far to easy for my liking. Have re-fitted clutch and new clutch cable all tightened but still the same.

Any ideas.. :?

Gear cable(s) too tight - pulling against each other when you want them to be still.
Gear worn or chipped - inside where it runs on layshaft/selector - or, if your American that's selectore!
Gear box incorrectly shimmed - gear next to the one your having trouble with pulling it out.

It won't be the clutch.
